You must use the correct tools!
Scissors: Dollar store folding sewing scissors are perfect for this. They have exactly the right point that can get in there without stabbing your flesh, they're of the just right stiffness, and they're easy to clean/disinfect. The fingerholes are uncomfortable, though.
Needlenose pliers: Important: Must be good quality. None of this crap where the end doesn't match up right or where the teeth stop before the end and it doesn't close completely.
Neosporin or reasonable facsimile:

Admittedly, sometimes I use the end of clippers instead of the scissors, if I'm lazy.
