The form on the site doesn't sign you up for anything, it just asks questions and eventually brings you to this page: http://www.makinghomeaffordable.gov/modification_yes.html

The site doesn't do anything for you, it just gives you information. You contact your lender and they tell you if they're participating in the program or not.
